These services do not replace individualized medical nutrition therapy from a registered dietitian when such care is indicated. Complete required forms, bring identification, insurance information, medication bottles or an updated medication list, and relevant health records.
Visit
Meet with the care team for a review of your concerns, medical history, medications, and health priorities.
Plan
Receive an individualized plan that may include treatment, testing, medication recommendations, education, monitoring, or referral.
Follow Up
Continue care through planned follow-up visits, laboratory review, medication monitoring, and preventive care.
