Managing Diabetes Requires More Than a Prescription

Diabetes is one of the most common and consequential chronic conditions in Mississippi — and one of the most manageable when treated consistently by a physician who knows your full picture. Dr. Olaleye provides comprehensive diabetes care that goes beyond A1C numbers: evaluating the metabolic drivers of your condition, coordinating diabetes-related care (eye exams, podiatry, nephrology), and adjusting your treatment plan as your health evolves.

What We Offer

Type 2 Diabetes Management

Comprehensive management including A1C monitoring, medication management (oral agents, insulin, GLP-1 agonists), lifestyle counseling, and ongoing plan adjustments.

Prediabetes Intervention

Early identification and structured intervention for patients with prediabetes — including lifestyle modification, metabolic monitoring, and medication when appropriate.

GLP-1 Receptor Agonist Therapy

When medically appropriate, Dr. Olaleye may prescribe GLP-1 receptor agonists such as semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound) for diabetes and weight management.

Insulin Management

Initiation, titration, and ongoing management of insulin therapy for patients requiring it — with education on self-monitoring and injection technique.

Metabolic & Lab Monitoring

Regular A1C, fasting glucose, kidney function, lipid panels, and other metabolic labs — tracked over time to guide treatment decisions.

Diabetes-Related Care Coordination

Coordination of diabetes-related specialty care — including annual dilated eye exams, podiatry referrals, nephrology for kidney disease, and cardiology when indicated.

Nutrition & Lifestyle Counseling

Evidence-based dietary and lifestyle guidance tailored to your metabolic profile — not generic handouts.

Complication Monitoring

Screening and monitoring for diabetes complications — including neuropathy, nephropathy, retinopathy, and cardiovascular risk — as part of ongoing care.

Why It Matters

Mississippi has one of the highest rates of diabetes in the country. Managing it well — not just keeping A1C below a threshold, but addressing the full metabolic picture — is what prevents the complications that change lives. That is what PHCM is here to do.

PHCM evaluates and manages many common adult medical conditions. Depending on the complexity of the condition, patients may also be referred to an appropriate specialist for consultation or co-management.

Common Questions

Frequently Asked Questions

Type 1 diabetes is an autoimmune condition in which the body produces little or no insulin and requires daily insulin therapy. Type 2 diabetes is the most common form and occurs when the body doesn't use insulin effectively. Dr. Olaleye manages adult patients with Type 2 diabetes, prediabetes, and insulin-dependent diabetes as part of a comprehensive internal medicine practice.

For most patients with diabetes, A1C should be checked every 3 months until the target is consistently met, then every 6 months. Patients with prediabetes or those newly diagnosed may be tested more frequently. Dr. Olaleye will establish a monitoring schedule based on your individual situation.

Type 2 diabetes can sometimes be put into remission — particularly with significant weight loss, dietary changes, and increased physical activity. Dr. Olaleye's physician-guided Metabolic Reset™ weight management program is designed to address the metabolic root causes of Type 2 diabetes alongside standard medical management.

Fasting glucose and fasting lipid panels require 8–12 hours of fasting. A1C testing does not require fasting. When you schedule your lab visit, our team will let you know whether fasting is needed for your specific tests.

Please bring your current medications and dosages, your blood glucose log or glucometer if you monitor at home, any recent lab results, and records from previous providers. The more information Dr. Olaleye has at the first visit, the faster he can build an effective care plan.
