PHCM evaluates and manages many common adult medical conditions. Depending on the complexity of the condition, patients may also be referred to an appropriate specialist for consultation or co-management.

Your first visit is a comprehensive new patient evaluation. Dr. Olaleye will review your full medical history, current medications, allergies, and any records from previous providers. He will perform a physical examination, order any appropriate lab work or imaging, and work with you to establish a care plan. Plan for approximately 45–60 minutes.
If Dr. Olaleye anticipates ordering a fasting metabolic panel or lipid panel at your first visit, you may be asked to fast for 8–12 hours beforehand. When you schedule, our team will let you know whether fasting is recommended for your specific visit type.
Yes. Internal medicine is specifically designed for ongoing management of adult chronic conditions. If you are transferring care from another provider, please bring your most recent records, lab results, and a current medication list so Dr. Olaleye can continue your care without interruption.
Yes. Telehealth visits are available for established patients for appropriate follow-up appointments, medication management, and minor concerns. New patients are seen in person for their initial evaluation. Contact our office to determine whether your visit qualifies for telehealth.
Referral requests can be submitted through our contact form or by calling either clinic directly. Dr. Olaleye will review your request and coordinate the referral. Some insurance plans require prior authorization, which our office will handle on your behalf.
