Medicare Annual Wellness Visits (AWV)

The Medicare AWV is a covered benefit focused on health risk assessment, preventive planning, advance care planning, and care coordination. Distinct from a standard physical, it is designed to help Medicare beneficiaries stay ahead of health risks.

Preventive Screenings

Age- and risk-appropriate screenings for cardiovascular disease, diabetes, kidney disease, and other conditions — with referral coordination for cancer screenings such as colonoscopy and mammography.

Immunizations

Review and administration of recommended adult vaccines — including influenza, pneumococcal, shingles (Shingrix), Tdap, and others based on age and risk factors.

Cardiovascular Risk Assessment

Evaluation of blood pressure, cholesterol, blood sugar, weight, family history, and lifestyle factors to calculate and address your cardiovascular risk.

Lifestyle & Risk Factor Counseling

Guidance on nutrition, physical activity, smoking cessation, alcohol use, and other modifiable risk factors — integrated into every wellness visit.

Medication Review

If you come in for a wellness visit and also raise a new medical concern, that concern may be billed separately as a problem-oriented visit.
